We are conveniently located across from Banner Estrella Medical Center within Bella Vida TMS. We are happy to offer free parking for our patients.
Our regular office hours are Monday – Friday, 7 am – 4 pm. We are closed for major holidays.
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Balanced Mental Health of Arizona, and they will contact us.
If you have an urgent after hours psychiatric need, please dial 988 for the suicide and crisis hotline.
At Balanced Mental Health of Arizona, we accept Tricare West, United healthcare, Blue Cross Blue Shield, Aetna , Cigna, and we accept Medicare as well. Other insurances are currently pending. Please contact your insurance company, or you may call our office at (623) 349-1711 to find out if we accept your plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience, we accept cash and all major credit cards, including HSA/FSA.

Prescriptions and refills for established patients of Balanced Mental Health of Arizona are routinely issued during our regular office hours (see above). You can make your request by calling our office at (623) 349-1711.
Refills are generally not issued during the evenings, weekends, or holidays, as the providers may not have access to all your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
